Laparoscopic Ovarian Drilling In Clomephene Citrate Resistant Cases Of Polycystic Ovarian Syndrome: An Original Research

Background: Polycystic ovary syndrome (PCOS) is one of the most common female endocrinopathies, with an incidence of 5-10% in women of reproductive age. The etiology, pathogenesis, and even treatment of infertile women who suffer from PCOS are still the subject of much controversy.
Material & Methods: In this interventional study 50 samples from O&G Department, VIMSAR, BURLA, SAMBALPUR from November 2019 – October 2021 have been collected. Preoperative routine investigations were done and preanaesthesia clearance was taken. Special attention is given to the patients for PCOS with clinical features of menstrual irregularities, hyperandrogenemia, having pylycistic ovaries, who are CC resistant patients posted for ovarian drilling.
Results: This study was undertaken to assess the clinical, endocrine profile and ovulation rate following LOD in clomefene citrate(CC) resistant PCOS cases .Rtterdam”s criteria was taken to define PCOS.LOD was done by monopolar electrocautery .Various literatures have been reviewed regarding the outcome in cases of CC resistant PCOS and compared with the outcome of this study. Following the procedure the patients were followed up for 1 year to observe the outcomes related to fertility.
Conclusion: Laparoscopic ovarian drilling is currently recommended as a safe, efficacious and cost-effective alternative to gonadotropins for ovulation induction in infertile, anovulatory, CCresistant PCOS women without th e risks of OHSS or multiple gestation Our study demonstrates significantly improved ovulation and pregnancy rates in drug resistant PCOS women after LOD. Some patients also did not achieve desirable results after the surgery
