Abstract

Aim and objectives: We conducted a randomised control prospective trial to compare Dexmedetomidine and Dexamethasone to Bupivacaine for Supraclavicular Brachial Plexus Block in Patients Undergoing Upper Arm (Below Shoulder) Surgery Using Peripheral Nerve Stimulator."

Materials and method:This prospective randomized clinical study was conducted after clearance from Board of Studies and Ethical committee in the Department of Anesthesiology, Muzaffarnagar Medical College, Muzaffarnagar (U.P).The sample was randomly divided into 2 groups: Group DX:- 0.5% bupivacaine (20ml) with dexamethasone (2ml) and normal saline(3ml) total 25ml.Group DM : - 0.5% bupivacaine (20ml) with dexmedetomidine(0.5ml) and normal saline (4.5ml) total 25ml.

Results: The Duration of Analgesia (minutes) was significantly more among Dexmedetomidine group (474.54±14.17 minutes) compared to Dexamethasone group (349.90±11.45 minutes). The mean VAS score at all time intervals post-operatively was significantly more among Dexamethasone group compared to Dexmedetomidine group. The mean Time to Rescue analgesics used all time intervals post-operatively was significantly more among Dexamethasone group compared to Dexmedetomidine group.

Conclusion:Both dexmedetomidine and dexamethasone are useful in the role of adjuvant while doing peripheral nerve blocks. But Dexmedetomidine is a superior choice for reducing the time it takes for motor block to take effect, as well as improving the quality and duration of supraclavicular block while maintaining a safe profile.
